Registration Checklist

Please make sure you complete the following forms and bring them with you at registration.

q  2009 CTYFL Completed Registration

q  2009 Physician’s Consent to Play Sports Form (must have physician’s signature and stamp)

Please provide us with the following information

q  Copy of legal Birth Certificate

q  Copy of 2x2 photo of player

q  Copy of recent report card

Please bring CASH for payment

q  $100.00 for program fee before July 17, 2009 (football & Cheerleading)

q  $120.00 late fee for registrations after July 17, 2009 (football & Cheerleading)

There will be absolutely no refunds.  If your child/children drops for any reason, it will be considered a donation to the Raiders Organization.
